Strange works fine for us. Add www. to your address for the image link (not sure if that will make a difference). Did you place the code in the head section on its own?

To get it to offer the review option to the customer i had to fill in the fulfilment days for the shipping methods in the shipping editor. Hope this helps.

Think this is it - Template editor / Header - then remove the background image, that should loose the black background. Then choose you colour in the Account Header.

I will try to remember how we did ours.
